1. Log onto Missouri Branch of Revenue's website (examine the link in the Reference divide below). Take the "Forms & Manuals" link tab located in the upper left corner.


2. Enter your zip code or city into the box. Select the "Go" button. This will provide a list of offices with addresses, hours of operation and phone numbers.


Select the "Commercial_Driver_License" link to retrieve Missouri's CDL booklet. Review it or print it out for future reference.

4. Log onto Missouri's DOR website to locate a license office. Find the "Locate a license office" tab in the upper right portion of the window.


5. Click on the downward facing arrow under "Sort" in the Centre of the event. Scroll through the drop-down menu and select "Drivers License--All." Click "Search Forms."3.


6. Visit your chosen office and request a CDL booklet from the customer service representative.
